2014-1 학교보험료 환불 신청. Application for a refund of school health insurance fee

2014-1 학교보험료(의료보험료) 환불 신청 (타보험가입자 해당)
기존 재학생 중 학교 외부 타보험에 가입하였지만 2014-1 학교보험 납입 제외 신청(기 공지글 참조. 신청기한 1/29까지였음)을 못 하여 2014-1 등록(등록금 납부) 시에 학교 의료보험료(55,610원)를 수업료와 함께 납부하였거나,
2014-1 신입생 중 학교 외부 타보험에 이미 가입되어 있어 2014-1 등록(등록금 납부) 시에 학교 의료보험료(55,610원)를 수업료와 함께 납부하여,
학교 의료보험료 55,610원을 환불 받고자 하는 학생은 아래 방법으로 환불 신청하여 주시기 바랍니다.
[신청방법] 아래 관련 서류를 2014.03.19(수) 오후 5시까지 외국인지원센터
이메일(ciss@khu.ac.kr) 또는 방문(외국인지원센터; 청운관 지하 1층 113호(B113)) 제출
- 서류 제출 시에 학번or수험번호/학과/이름(영문)/연락처/타보험 보험적용기간 필수 기재
[제출서류] 1. 타보험 증서 사본
2. 등록금 납부 영수증 (의료보험료 납부 내용 포함)
3. 통장 사본 (본인 명의의 통장)
※ 2014.03.02~2014.08.31에 보험적용이 가능한 타보험이어야 하며, 타보험 증서 제출시에 보험 적용 가능 기간을 반드시 알려주시기 바랍니다.
만약, 보험적용기간이 2014.03.02~2014.08.31을 전일 포함하지 않고 부분적으로만 포함하는 타보험이라면, 학교보험에 가입하지 않을 경우 보험에 미가입 하게 되는 공백이 생기므로, 이런 경우의 타보험 가입자는 학교보험에 가입하여 주시기 바랍니다.
※ 외국인 유학생의 의료보험 가입은 의무 사항입니다.
외국인 유학생은 의료보험(학교보험 또는 학교 외부 타보험)에 가입되어 있어야 교내 장학금 및 프로그램 신청 가능한 점 참고해주시기 바랍니다.
※ 학교보험 환불금(55,610원)은 4월 중에 환불/송금 처리 예정입니다.

Application for a refund of school health insurance fee
Students those who have registered in other health insurance companies (except school health insurance) for 2014-1 can receive a refund of school health insurance fee (55,610)
[How to apply] Please submit required application materials by e-mail(ciss@khu.ac.kr) or in person(the office of CISS(Center for International Students & Scholars; Cheongwoon Bldg. #B113(next to the Global Zone)) by 19th of March, 5pm
- Student number or Application number/Major/Name(English)/Cell Phone No./Policy Period(other insurance) should be stated in e-mail or materials.
[Requirements] 1. A copy of insurance certificate of other companies
2. A receipt of tuition payment (including school health insurance fee)
3. A copy of bank book of one’s own account
※ The policy period of other health insurance should be valid from 2014.03.02 to 2014.08.31. Please send the e-mail with the policy period of other health insurance or inform us the policy period when you visit the office of CISS.
Students those who don’t have any other health insurance for international students should pay school insurance fee with tuition fees.
※ International Students MUST have Health insurance (of school or other companies).
Students MUST have insurance to participate in all programs and apply for scholarships of Kyung Hee University.
※ The School Health insurance fee will be refunded to your bank account in April.
